To ensure the health and safety of students in a laboratory setting, by May 2018, San Diego should remind all departments to retain student training acknowledgment forms for at least three years after the end of classes.
1-Year Agency Response
In adherence to CSU Executive Order 1039, campus colleges and departments were reminded that student training acknowledgement forms should be kept on file for at least three years.

San Diego provided evidence demonstrating that it reminded all departments to retain student training acknowledgment forms for at least three years after the end of classes.
6-Month Agency Response
An email was distributed by the Associate Director, Environmental Health and Safety, to the Associate Deans which reminded the colleges and departments to retain student training acknowledgement forms for at least three years after the end of classes. A sample acknowledgement form was also attached.

Until San Diego can sufficiently demonstrate that it requires departments to have those students required to wear PPE sign the student safety training acknowledgement forms, we will continue to report this recommendation as not fully implemented.
